She/he will provide in-home and facility supervision to Registered Nurses (RN) and Licensed Vocational Nurses (LVN)/Licensed Practical Nurse (LPN) every thirty (30) days and to Caregivers (PCA/Companions/Live-ins/Homemakers) every sixty (60) days, and complete appropriate documentation per Caring People policy.

QUALIFICATIONS (Must Haves) Currently licensed Registered Nurse in good standing, in the State in which services are provided.
Graduate of an accredited school of nursing.
COVID VACCINATED AND BOOSTED At least two-years of experience in an acute care setting, one of which is in a home healthcare setting is preferred.
Current CPR certification and must maintain certification.
Maintain nursing license by obtaining the necessary contact hours and competency.
Physically capable and able to lift and transfer clients to the level and amount determined by each individual client to be able to care for clients in a home setting.
Possess effective communication skills and be able to follow instructions to complete work as scheduled.
Active and current driver's license and have a dependable insured automobile.
Must be comfortable with significant daily driving to and from client residences.
Possess excellent people and customer service skills with excellent communication skills, both oral and written.
Reliable and have a positive attitude, which fosters a cooperative and positive atmosphere.
Neat in appearance.
RESPONSIBILITIES The following duties and responsibilities reflect the essential functions for this job, but do not restrict the tasks that may be assigned.
The Director of Nursing or Regional Director of Operations may assign or reassign duties and responsibilities to this job at any time due to reasonable accommodation or for other reasons.
Performs the essential activities including assessment, planning, implementation, coordination, monitoring, evaluation, outcomes, and general interventions across case management practice and processes, and makes appropriate referrals to other disciplines.
Performs initial evaluation and assessment of client needs and contacts physician for initial/continuing orders Establishes the Care Plan with team and coordinates, implements appropriate alternatives, and makes a determination of expected client outcomes and goals.
Updates and revises the Care Plan as necessary.
Makes appropriate referrals for therapies and medical equipment/supplies based on client needs.
Observes client for changes in condition and obtains orders for changes to the Care Plan, documents all observations and interventions in the clinical record in a timely manner.
Develops and keeps current orders for medications and treat approval as appropriate.
Uses appropriate preventive, therapeutic, and rehabilitative nursing procedures and keeps client's physician, family and the Nursing Supervisor informed of care being provided.
Assists and instructs client and family members in nursing care required to promote client's health and safety.
Documents nursing observations, interventions and evaluations of the care being provided in accordance with agency policy.
When assigned, supervises ancillary personnel.
Participates in team conferences, as requested, and attends staff in-services programs.
Provides Nurse Delegation to appropriate staff and supervises same to assure compliance with the delegation and safety of the clients.
Thoroughly explains the Company's mission and value to the client and client's family members, doctors, providers, etc.
Explains role and services of the Company, describes client's rights, and provides contact information for complaints.
Understand and respond to a diverse population.
Sit and drive for long periods of time.
Physically capable of walking, climbing stairs, lifting up to 50 lbs; transfer of another individual.
Required to talk and hear.
Required to be able to communicate electronically.
SUPERVISORY RESPONSIBILITIES Provides supervision, support, and instruction to RNs, LVNs, LPNs, PCAs, CHHAs, CNAs, Companions.
Is actively engaged in the training and certification of other Registered Nurses.
SCHEDULE RESPONSIBILITIES Full time, salaried position.
Weeknight & weekend on-call rotating responsibilities, as needed.
